What is a Substance Use Evaluation?
A substance use evaluation is a professional assessment that examines your relationship with alcohol and drugs. It provides an objective analysis of your substance use patterns, identifies potential concerns, and offers recommendations for treatment or monitoring if needed. After a thorough assessment is completed, you will receive a detailed written report that can be submitted to courts, attorneys, probation officers, or other entities requiring this information.
Who Needs a Substance Use Evaluation?
You may need a substance use evaluation if you’re involved in:
Legal proceedings such as DUI/DWI cases, criminal court requirements, or probation compliance.
Family court matters including custody disputes, divorce proceedings, or child protective services cases.
Employment/Academic situations where your employer, licensing board, or educational institution requires documentation of your substance use history.
Personal assessment to better understand your relationship with substances and determine if treatment would be beneficial.
Evaluation Process
Step 1. We set up a consultation phone call to discuss your specific situation, how we can best help you, and to schedule a time for your interview. It is imperative that you let us know your time frame for when the report is needed from the onset. For those who need an expedited report there is an additional fee.
Step 2. We meet either in person or virtually for a 60-90 minute interview. Wanting to provide the most accurate information in the report, you can expect your interview to consist of a series of questions aimed at gathering as much information as possible.
Prior to your interview, you are required (if court ordered) to complete a 12-panel urine drug screening with a third party establishment. You must bring or send your results to me, unaltered and sealed, so that it can be included in your evaluation.
Step 3. Once the interview is complete, we compile the gathered information into a report and send it to you to review before finalizing the document.

The evaluator is committed to documenting the examinee’s statements as accurately as possible. However, the evaluator is not liable for any information contained in the evaluation report that may differ from the petitioner’s testimony, statements, affidavits, or other communications provided in legal or administrative proceedings. By proceeding with this evaluation, you acknowledge that the report reflects the evaluator’s clinical findings and professional judgment based on the information gathered during the assessment process. The evaluation is an independent clinical document and does not guarantee any particular outcome in legal, employment, or family court matters.
